You declare that you are aware of the risks, care required, and the healing process of the tattoo.

Legal protection

The statement confirms that the procedure was performed with your authorization, avoiding misunderstandings.

Health and safety

Please inform us of any allergies, medical conditions, or medications you are taking that may interfere with healing.

Minimum age

Tattooing minors under the age of 18 is considered a crime of bodily harm throughout the State of São Paulo, even with the authorization and/or presence of a legal guardian.

Post-tattoo care

You confirm that you have received all the necessary instructions to ensure the best possible healing of your tattoo.

Image Usage

With your consent, I can use photos of your tattoo for my portfolio and social media.

If you prefer not to authorize it, just let me know.

With the completed and signed form, you're ready to get your tattoo!

You will receive a copy of the document in your email containing all your information, a detailed text about the aftercare of your tattoo, and a complete list of supplies used in the procedure (such as cosmetics and pigments).
